Ncore · EMR

The first value-based care EMR with complete front office support.

For physicians & practice owners

Every EMR was built to document visits. Ncore is built to run a panel: notes that draft themselves, coding that happens automatically, a front office the AI operates — and proactive outreach that manages patients between visits, so each provider can safely oversee more of them.

The newest module of the platform — built from the ground up around the AI, the phones, and value-based care. See it early: it's the last migration your practice will ever do.

Documentation that writes itself.

The chart work that eats your evenings is exactly what AI is best at. Ncore drafts the note, suggests the code, and preps the orders — you review, adjust, and sign. The design target is ruthless: the fewest clicks in the industry between seeing a patient and a signed, coded, billed encounter.

  • AI-drafted SOAP notes from the visit — edit, don't type
  • Automatic coding suggestions — E/M level and diagnoses proposed with the note, reviewed by you
  • Refill auto-approval under your protocols — routine renewals clear themselves; exceptions surface
  • An inbox that works itself down — results, messages, and tasks triaged and drafted before you open them

A front office that runs itself.

Ncore ships with the platform's voice AI built in — which means the EMR doesn't stop at the exam room door. The phones, the schedule, the intake, the reminders, and the coverage checks all run inside the same system, operated by AI, overseen by your staff.

  • Patient calls answered 24/7 — scheduling, questions, and messages handled in conversation
  • Intake before arrival — history, forms, and consents collected by SMS and voice ahead of the visit
  • Eligibility checked automatically — Nsure runs inside the schedule, so coverage surprises don't reach the visit
  • No-show recovery — reminders, confirmations, and same-day rebooking without staff touching the phone

Built for value-based care: the EMR manages the panel, not just the visit.

Fee-for-service EMRs wait for patients to show up. Ncore triages your panel automatically and works it proactively — checking in with patients over SMS and AI voice, running assessments, and bringing the right patients in before problems compound. Every interaction lands with the provider as a recording and a structured note, with care plan adjustments already recommended for review.

  • Automatic panel triage — every patient continuously scored, so the sickest surface first
  • Proactive check-ins via SMS + voice — assessments between visits, not just at them
  • Brings patients in proactively — or recommends the care plan adjustment, with the recording and note pushed in for provider review
  • Larger panels per provider, safely — oversight by exception instead of visit-by-visit discovery

Connected to the rest of the platform on day one.

Ncore is shell-native: the same login, the same patient record, and the same design language as every other module. Sign a note and the encounter routes to Nvera for billing. Schedule a visit and Nsure verifies coverage. Enroll a patient and Nsight starts the program. Nothing to integrate — it's one system.

  • Sign → billed — encounters flow to the revenue cycle without an interface engine
  • Schedule → verified — eligibility runs automatically on every booked visit
  • Runs standalone too — Ncore is a complete EMR on its own; the suite makes it stronger, never dependent

Ncore, answered.

When can we get it?+
Ncore is the newest module of the platform and is rolling out to early practices first. Schedule a Demo to see where it stands, what's in the current build, and where your practice would fall in the rollout — we'll give you real dates, not vaporware promises.
What does migration from our current EMR look like?+
Structured data — patients, problems, medications, allergies, histories — maps into the platform's patient index, and documents come across as part of onboarding. Because the rest of the suite integrates with your existing EMR today, most practices adopt other modules first and move the chart last, on their own timeline.
Does the AI decide anything clinically?+
No. Ncore is propose-and-review by design: notes, codes, triage scores, and care plan adjustments are drafts and recommendations until a clinician signs them. Refill auto-approval runs only under protocols your providers define, with every action logged and auditable.
Why build a new EMR at all?+
Because bolting AI onto a 20-year-old chart gets you a 20-year-old chart with a chatbot. Panel triage, proactive outreach, an AI front office, and sign-to-billed automation only work when they're the architecture, not an add-on — and that's a rebuild, not a plugin.

See the EMR that manages the panel — not just the visit.
